Key Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans, taking into account the unique needs and preferences of each resident
- Collaborate with the healthcare team to ensure seamless care transitions and effective communication
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices
- Provide education and support to residents and their families, promoting health literacy and empowerment
- Current RN licensure in the state of Ohio
- Minimum 1 year of experience in long-term care or a related field
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
- Ability to lift, transfer, and position residents, as needed
- Basic Life Support (BLS) certification
