Admissions Nurse Specialist

About the Role
As an Admissions Nurse at Bethesda Health, you will play a vital role in ensuring a seamless transition for our patients and their families. Your primary responsibility will be to complete the admission process, which includes assessing, planning, implementing, and evaluating patient care plans. You will welcome patients, assist with the move-in process, and deliver a Welcome Packet, answering questions and initiating the C.A.R.E.S. process. You will also coordinate communication with families, nursing home staff, social workers, and the team regarding the admission process. Additionally, you will complete initial plans of care in the Electronic Medical Record (EMR), conduct head-to-toe assessments, and ensure that all necessary treatments and orders are in place. You will also add any wounds to the weekly wound log, initiate preventative measures, and complete all necessary admission paperwork, including initial plans of care, medication lists, admit papers, physician orders, and code status papers. You will verify orders with physicians, obtain and enter patient immunization status into the EMR, and offer/administer/document immunizations as needed. You will demonstrate familiarity with agency policies and regulatory requirements. This role requires a graduate of an accredited school of nursing, a current nursing license in good standing, and a minimum of two years of varied work experience as a professional nurse, preferably in a post-acute setting. You will possess excellent administrative performance skills, including expertise in written and oral communication and documentation requirements. You will also demonstrate the ability to assess and respond to patient and family needs in various settings, possess nursing practice skills, and be able to lift, transfer, and/or move items or patients with weights of 10-200+ pounds. You will be able to perform critical thinking and psychosocial assessments and judgments, and possess manual dexterity and skillful use of hands when working with patients and equipment.
