Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Alden Debes Rehabilitation and Health Care Center. As a LPN, you will provide direct nursing care to our customers and supervise the day-to-day nursing activities performed by our nursing assistants.

Responsibilities
  • Provide direct nursing care to customers, including administering medications, performing routine charting duties, and participating in the orientation of new customers and family members.
  • Supervise and instruct nursing assistants on a variety of topics to promote job knowledge and ensure adherence to established departmental policies and procedures.
  • Develop and implement written plans of care for each customer, identifying problems and needs, and attending and participating in customer care plan conferences as directed.
  • Report concerns, complaints, and incidents to the Administrator, and counsel and discipline nursing personnel as necessary.
  • Make daily rounds to ensure nursing personnel are performing required duties and to ensure appropriate procedures are being followed.
  • Collaborate with our CNA's who provide ADL care for our residents as well as work hand in hand with their nurses to provide weights, vitals, skin care and change in resident behavior/status information.
  • Participate in the Quality Assurance Performance Improvement process to ensure we are getting better.
Requirements
  • Must possess a current, unencumbered license to practice as a LPN in the State of Illinois and/or Wisconsin.
  • Must possess current CPR certificate or become certified within two weeks of employment.
  • Must be knowledgeable of nursing and medical practices and procedures.
  • Must be able to read, write, and speak the English language in an understandable manner.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with customers, family members, visitors, personnel, government agencies/personnel, and the general public.
  • Must possess leadership and supervisory ability and willingness to work harmoniously with other personnel.
Physical Requirements
  • Must be able to move intermittently throughout the workday.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with customers, family members, personnel, and support agencies.
