Job Summary

The Occupational Therapist is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement. This role respects and promotes the mission, values, and vision of VNA Health.

Key Responsibilities
  • Evaluate patient's functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity.
  • Assess home environment for hazards or barriers to independent living and identify equipment needs.
  • Participate in the development of the total plan of care.
  • For patients returning to work, perform work capacity evaluations and refer to specialized vocational training in accordance with Organization policy.
  • Develop treatment programs and establish goals for improved function.
  • Communicate plans of care to referring physicians and other Organization professionals.
  • Teach new skills or retrain patients in daily activities lost due to illness or injury, in accordance with Organization policy.
  • Train patients in the use of prosthetic and/or orthotic devices.
  • Maintain clinical records, clinical notes, and reports to physicians any changes in patient condition.
  • Attend in-services, case conferences, and other meetings as required by Organization policy to ensure coordinated and comprehensive plans of care.
  • Knowledge of state, federal, local, and accreditation regulations for the delivery of home health services.
  • Instruct patient's family/caregivers and other Organization healthcare personnel in patient treatment regimes when indicated and appropriate.
  • Participate in discharge planning for patients.
Qualifications
  • Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Two years of clinical experience as an occupational therapist.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possess and maintains current CPR Certification.
Physical Requirements
  • Ability to lift 50 lbs.
  • Ability to push/pull 50 lbs.
  • Ability to stoop, bend, and squat.
  • Ability to walk up one flight of stairs.
  • Ability to walk one mile.
  • Ability to sit for one hour, minimum.
  • Visual and audio acuity.
