About the Role

Avera is seeking a skilled Social Worker to join our team. As a Social Worker, you will provide medical social casework to patients and families, assisting in meeting their psychosocial and continuum of care needs.

Key Responsibilities

  • Provide direct care services to patients and families, including assessment, planning, and implementation of care plans.
  • Consult with healthcare professionals to ensure comprehensive care.
  • Develop and implement policies and programs to improve patient outcomes.
  • Collaborate with community resources to provide support and services to patients and families.
  • Supervise and peer social work consultation.

Requirements

  • Master's degree in Social Work required.
  • Current Social Work license in the state of Nebraska required.
  • Valid driver's license and insurability by Avera Health automobile liability insurance carrier.

Work Schedule

Monday-Friday, 8:00am-4:30pm; 80hrs/2wks

Pay

The hourly range for this position is $28.75 - $43.50 per hour.
