Speech Language Pathologist Home Health Per Diem Opportunity

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a key member of our care team, you will work with patients in their homes to evaluate, direct, and provide speech/language pathology services.
Responsibilities- Evaluate patients' communication and swallowing abilities, and develop personalized treatment plans to address any deficits or disorders.
- Conduct assessments, administer diagnostic tests, and interpret results to inform treatment decisions.
- Provide individualized therapy sessions to patients, focusing on speech, language, voice, hearing, and swallowing skills.
- Collaborate with other healthcare professionals to ensure comprehensive care and effective communication.
- Document patient progress, maintain accurate records, and submit reports as required.
- Participate in care coordination activities, discharge planning, and patient education to promote optimal outcomes.
- Master's degree in Speech-Language Pathology or related field.
- Current licensure and certification in Speech-Language Pathology.
- Minimum of six months experience as a speech therapist or speech-language pathologist.
- Home Health experience preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
