Job Summary:
Mercy Hospital is seeking a skilled and compassionate Home Health Nurse to join our team. As a Home Health Nurse, you will provide high-quality patient care in the comfort of patients' homes, ensuring their safety and well-being.
Key Responsibilities:
- Provide day-to-day patient care in the home setting, including case management, total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care, and general skilled nursing according to policy and procedure.
- Assist with other needs that may be apparent in the home to assure safety and appropriate care, such as lifeline, meals on wheels, and instruction on public programs availability.
- Provide appropriate and timely documentation by utilizing the Epic computer system.
Requirements:
- Nursing Degree from an accredited school
- Oklahoma RN licensure
- Valid driver's license
- Certifications: CPR certification, Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire, LPN-IV Certification must be previously completed or successful completion of course within 3 months of hire
- Prefer one year experience in a related field
