**Responsibilities:**

* Conduct thorough Start of Care (SOC) Admissions - OASIS
* Develop and implement personalized care plans tailored to each client's unique needs
* Provide skilled nursing care, including administration of medication, wound care, treatments, and procedures
* Monitor clients' conditions and report changes to Clinical or Client Services Manager
* Execute and properly document doctors' orders
* Perform client assessments and accurately document observations, interventions, and evaluations

**Qualifications:**

* Current license as a Registered Nurse in New York State
* Minimum of two years of recent, verifiable clinical (medical/surgical) experience
* Prior home care and OASIS experience required
* Graduation from an accredited and approved nursing program
* Ability to work independently and manage time effectively
* Strong interpersonal skills
* Solid computer skills, prior experience with electronic medical records (EMR) preferred
