About the Role

We are seeking a highly skilled and compassionate Senior Nurse Practitioner to join our team at CenterWell Senior Primary Care. As a key member of our care team, you will play a vital role in delivering high-quality, patient-centered care to our seniors.

Key Responsibilities
  • Evaluate and treat patients in accordance with established standards of care.
  • Collaborate with our multidisciplinary care team to coordinate patient services, including specialty referrals, hospital and SNF coordination, and durable medical equipment.
  • Act as an active participant and leader in daily care team huddles, ensuring seamless communication and coordination of patient care.
  • Complete all medical record documentation in a timely manner, working closely with our quality-based coder to optimize coding specificity.
  • Follow policy and protocol defined by Clinical Leadership, ensuring compliance with licensing, certification, and accrediting bodies.
Requirements
  • Minimum of two to five years directly applicable experience as a Nurse Practitioner.
  • Successful completion of a Nurse Practitioner program with Board Certification.
  • Active, unrestricted Nurse Practitioner license in the state of practice location.
  • Excellent communication skills, with a high level of skill in interpersonal relationships and communications with all colleagues.
  • Willingness and ability to learn/adapt to practice in a value-based care setting.
  • Superior patient/customer service skills.
  • Basic computer skills, including email and EMR.
  • Appropriate certification to write prescriptions under the authority of a collaborating physician/medical director.
Preferred Qualifications
  • Primary Care Outpatient Experience.
  • Active and unrestricted DEA license.
  • Medicare Provider Number/Medicaid Provider Number.
  • Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
  • Knowledge of Medicare guidelines and coverage.
  • Bilingual is a plus.
  • Knowledge of HEDIS quality indicators.
