About the Opportunity

We are seeking a highly skilled and compassionate Senior Homecare Nursing Coordinator to join our team at Baystate Health. As a key member of our home health team, you will play a vital role in providing exceptional patient care and support to patients in their homes.

Responsibilities

As a Senior Homecare Nursing Coordinator, your primary responsibilities will include:

  • Coordinating patient care with our interdisciplinary team
  • Developing and implementing personalized care plans for patients
  • Providing education and support to patients and families on disease management and self-care
  • Collaborating with healthcare providers to ensure seamless transitions of care
Requirements

To be successful in this role, you must have:

  • A diploma in nursing and current licensure in the Commonwealth of Massachusetts
  • Current American Heart Association Health Care Provider CPR certification
