Key Responsibilities- Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
- Provide symptom management, emotional and spiritual support, medication management, education, and IDT oversight to patients and their families.
- Work autonomously and as part of a team to ensure seamless care coordination and communication.
- Manage a caseload of patients, prioritizing needs and providing timely interventions.
- Collaborate with physicians, social workers, and other healthcare professionals to ensure comprehensive care.
- Document patient information, progress, and outcomes in accordance with regulatory requirements.
- Current RN license in the state of practice.
- 2 years of RN experience, with Hospice or Acute Care experience preferred.
- Basic Life Support (BLS) certification.
- Valid driver's license and proof of auto insurance.
