Job Title: Home Health Nurse

Aveanna Healthcare is seeking a compassionate and skilled Home Health Nurse to provide high-quality care to our patients in the comfort of their own homes.

Job Summary:

We are looking for a dedicated and experienced RN to join our team as a Home Health Nurse. As a Home Health Nurse, you will be responsible for providing skilled nursing care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities:
  • Implement physician-prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medications, treatments, and therapies
  • Document care provided in our electronic system
  • Teach and train family members on health promotion and disease management
  • Communicate with physicians, clients, family members, and staff on patient concerns and changes in condition
Requirements:
  • Active, unencumbered RN license in the state of practice
  • CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • 6 months prior hands-on nursing experience preferred
  • Reliable transportation
