Job Summary: We are seeking a highly skilled Pharmacist to join our team as a Clinical Pharmacy Consultant. As a key member of our team, you will be responsible for providing clinical consultative services to patients and therapeutic recommendations to nurses, physicians, and other hospice personnel. You will also conduct and evaluate medication profiles/histories, assess compliance, and suggest modifications to optimize medication regimens using sound clinical pharmacy principles.

Salary: $58 - $62 per hour

About the Role: The ideal candidate will have a strong background in clinical pharmacy and experience working in a hospital or hospice setting. You will be responsible for providing expert clinical guidance to our patients and caregivers, while ensuring adherence to regulatory requirements and industry standards.

Responsibilities:

  • Provide clinical consultative services to patients and therapeutic recommendations to nurses, physicians, and other hospice personnel.
  • Conduct and evaluate medication profiles/histories, assess compliance, and suggest modifications to optimize medication regimens.
  • Collaborate with our team to develop and implement treatment plans that meet the unique needs of each patient.

Requirements:

  • Advanced degree in pharmacy (PhD or MS) preferred.
  • Experience working in a hospital or hospice setting.
  • Excellent clinical judgment and decision-making skills.
  • Strong analytical and problem-solving skills.
  • Current Pharmacist License in good standing.
