Job Summary:

The Diagnostic Clinic is seeking a skilled Healthcare Navigator to join our team. As a key member of our healthcare team, you will be responsible for managing the post-acute care of high-risk patients who are at risk for poor health outcomes, frequent emergency room visits, and hospital readmissions.

Key Responsibilities:

  • Facilitate clinical care, patients' access to appropriate services, and service referrals and appointments upon patient hospital or ED discharge.
  • Manage medication reconciliation and adherence, patient quality of life and functionality, acute and chronic disease states, and identify and rectify gaps in care.
  • Assess and support patients' ability to perform self-care, coordinate post-discharge appointments and services, and coordinate care across the care continuum.
  • Collect clinical path variance data to identify potential areas for system-wide improvement of care and services.

Requirements:

  • Minimum Bachelor's degree in Nursing (BSN) or equivalent combination of education and experience.
  • Minimum three years of relevant clinical nursing experience.
  • Current RN licensure and CPR certification.
  • Preferred certification as a case manager.
