Key Responsibilities:
  • Conduct comprehensive psychiatric assessments and develop personalized care plans in collaboration with the Interdisciplinary Team (IDT)
  • Partner with Mental Health Counselors to guide IDT meetings and empower the team with tools for effective interventions
  • Educate IDT members on mental health disorders, substance abuse, dementia-related behaviors, and goal setting for meaningful behavioral interventions
  • Integrate behavioral health into participants' care plans seamlessly, working closely with clinicians
  • Establish and maintain strong communication with community behavioral health specialists to ensure collaborative care
  • Manage schedules and facilitate the exchange of critical information to make the most of geriatric psychiatric consultations
  • Support participants admitted to psychiatric facilities to ensure issues are addressed, and follow-up care is well-planned
  • Evaluate participants with complex behavioral health issues to develop accurate diagnoses and tailored care plans
  • Provide behavioral health education to team members, equipping them with knowledge to enhance participant care
  • Contribute to the continued growth and success of the organization by embracing additional opportunities and responsibilities as needed
Requirements:
  • Passion for serving high-risk seniors and frail older adults
  • 3+ years of experience providing behavioral healthcare to elderly or complex populations
  • Experience working with Interdisciplinary Teams in a healthcare setting
  • Active Psychiatric Mental Health Nurse Practitioner State License without restrictions
  • State CDS Certification and DEA License
  • NPI License required
  • PACE experience preferred but not required
  • CPR and First Aid certification (or ability to obtain within 60 days)
  • Valid CA driver's license, personal transportation, good driving record, and auto insurance (as required by state law)
