Key Responsibilities:- Conduct comprehensive psychiatric assessments and develop personalized care plans in collaboration with the Interdisciplinary Team (IDT)
- Partner with Mental Health Counselors to guide IDT meetings and empower the team with tools for effective interventions
- Educate IDT members on mental health disorders, substance abuse, dementia-related behaviors, and goal setting for meaningful behavioral interventions
- Integrate behavioral health into participants' care plans seamlessly, working closely with clinicians
- Establish and maintain strong communication with community behavioral health specialists to ensure collaborative care
- Manage schedules and facilitate the exchange of critical information to make the most of geriatric psychiatric consultations
- Support participants admitted to psychiatric facilities to ensure issues are addressed, and follow-up care is well-planned
- Evaluate participants with complex behavioral health issues to develop accurate diagnoses and tailored care plans
- Provide behavioral health education to team members, equipping them with knowledge to enhance participant care
- Contribute to the continued growth and success of the organization by embracing additional opportunities and responsibilities as needed
- Passion for serving high-risk seniors and frail older adults
- 3+ years of experience providing behavioral healthcare to elderly or complex populations
- Experience working with Interdisciplinary Teams in a healthcare setting
- Active Psychiatric Mental Health Nurse Practitioner State License without restrictions
- State CDS Certification and DEA License
- NPI License required
- PACE experience preferred but not required
- CPR and First Aid certification (or ability to obtain within 60 days)
- Valid CA driver's license, personal transportation, good driving record, and auto insurance (as required by state law)

Base pay is based on several factors, including education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
