Job Overview

We are seeking a highly skilled Field Nurse Case Manager to cover the San Francisco area and surrounding locations. As a Field Case Manager, you will work from home and travel locally to doctors' offices, hospitals, and other locations to provide medical case management services for work-related injuries.

Key Responsibilities

  • Conduct thorough assessments, develop comprehensive plans, coordinate, implement, and evaluate the care of injured or disabled individuals.
  • Collaborate closely with insurance carriers, medical providers, attorneys, employers, and employees to ensure seamless care delivery.
  • Monitor progress and report results to employers and insurance carriers.

Requirements

To succeed in this role, you must possess:

  • A current, unencumbered Registered Nurse (RN) license in California.
  • Experience in case management of workers' compensation cases is preferred.
  • Recommended certifications such as CCM, CRRN, CDMS, CRC are also preferred.
  • Strong organizational and timeline adherence skills.
  • Proficiency in PC and technology skills.
  • Excellent communication skills (written, verbal, and interpersonal).
  • The ability to operate autonomously with minimal oversight.
  • Skilled in patient education.
  • A valid driver's license and good driving history.

Compensation and Benefits

We offer a competitive salary of $80,000 - $110,000 per year, depending on experience, plus benefits that include:
