Responsibilities:
- Conduct patient assessments and collaborate with the care team to formulate and execute a comprehensive care plan.
- Refer patients to other healthcare professionals as necessary based on identified needs.
- Encourage patient health and autonomy through education and rehabilitative strategies, assisting patients in mastering self-care techniques.
- Supervise LPNs and HHAs.
- Ensure timely and accurate documentation.
- Regularly update the clinical manager and care team on patient progress.
- Plan and deliver staff training.
- Fulfill on-call duties and provide on-call services to patients and families as required.
- Engage in clinical development and continuing education initiatives.
- Perform additional duties as assigned.
Qualifications:
- Minimum of one (1) year of clinical experience as a Registered Nurse (RN). Candidates with less than one year of experience must receive approval from the VP Clinical.
- Current RN license specific to the state(s) of employment.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Candidates with less than six months of clinical experience as an RN must participate in the RN Intern program.
