Our comprehensive approach includes a variety of programs such as Primary Care, HIV/STD Testing, PrEP, Community Outreach, Case Management, and a statewide Information Hotline, all aimed at reducing new infections and enhancing the quality of life for our clients.

Key Responsibilities:
As a Patient Advocate, your primary duties will include:
1. Actively assist patients in navigating and accessing HIV/AIDS treatment and support services.
2. Collaborate with clinic personnel to connect patients with necessary resources related to health insurance, case management, financial aid, and other essential services.
3. Identify barriers to care and provide support to patients and health center staff in overcoming these challenges.
4. Follow up with patients who have missed appointments and coordinate rescheduling efforts.
5. Educate newly diagnosed patients about HIV/AIDS and available resources.
6. Offer emotional support and refer patients to support groups and educational classes.
7. Document patient interactions accurately in the electronic medical record system.
8. Maintain confidentiality and adhere to strict data security protocols.
9. Participate in team meetings to enhance patient navigation services and clinic performance.
10. Engage in ongoing training to stay informed about current HIV-related information.
Qualifications:
To be successful in this role, candidates should possess:
- A high school diploma or equivalent.
- Familiarity with the healthcare environment related to HIV/AIDS services.
- Certification in HIV Testing & Counseling is preferred.
- Experience in community engagement and relationship building.
- Basic computer skills, including proficiency in Microsoft Office.
- Bilingual skills (English/Spanish) are highly valued but not mandatory.
- A commitment to maintaining confidentiality and professionalism in all interactions.
