Hospice Nurse Practitioner

LHC Group is seeking a compassionate and skilled Nurse Practitioner to join our Hospice team. As a key member of our interdisciplinary team, you will provide consultation in palliative care, symptom management, and supportive care to meet the needs of our Hospice patients.

Responsibilities:

* Provide face-to-face encounter visits to qualifying Hospice patients and report clinical findings to the Hospice Medical Director
* Consult with the Hospice Medical Director as needed and inform primary physicians of recommended services
* Initiate and coordinate the plan of care, documenting problems, goals, interventions, and patient/family response to hospice care
* Collaborate with patients, families, attending physicians, and other members of the IDG to provide patient and family care
* Instruct and supervise patients and families in self-care techniques when appropriate

Requirements:

* Minimum of five years nursing experience, with clinical nursing care in oncology, hospice, geriatrics, or intensive care preferred
* Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice
* Current certification by a national body such as ANCC, NBCHPN, or AANP
* Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation
* Current CPR certification required
