Job Summary

The GVN Graduate Vocational Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. As a key member of our healthcare team, you will be responsible for providing direct patient care, collaborating with our interdisciplinary team, and contributing to the overall success of our skilled nursing facility.

Key Responsibilities

  • Conduct thorough assessments of residents' physical, emotional, and social needs, and develop individualized care plans to address these needs.
  • Administer medications and treatments as prescribed by our medical team, and monitor residents' responses to these interventions.
  • Collaborate with our interdisciplinary team to develop and implement care plans that promote resident independence and well-being.
  • Provide education and support to residents and their families on health-related topics and disease management.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote resident safety and satisfaction.
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination and transitions.

Requirements

  • Graduate of an accredited vocational nursing program.
  • Currently licensed or eligible for licensure as a Licensed Vocational Nurse (LVN) in the state of Texas.
  • Experience working in a skilled nursing facility or similar healthcare setting preferred.
