About the Job

LHC Group is seeking a skilled Physical Therapy Assistant to join our team in providing high-quality care to patients in their homes.

The ideal candidate will have a strong background in physical therapy and be able to work independently with minimal supervision.

Responsibilities
  • Provide physical therapy services to patients in their homes, including exercises, stretches, and other interventions to improve mobility and function.
  • Work closely with the supervising Physical Therapist to develop and implement treatment plans tailored to each patient's needs.
  • Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination.
  • Document patient progress and maintain accurate records of treatment.
Requirements
  • Current licensure as a Physical Therapy Assistant in the state of practice.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
