  • Conduct in-home health assessments, prescribe medications, and develop treatment plans while exercising independent judgment in complex situations.
  • Engage with patients and their families, aiming to build strong interpersonal relationships and trust.
  • Document patient interactions and care plans using electronic medical records (EMR) systems.
  • Coordinate care management, including prescription refills and lab follow-ups, to enhance patient health outcomes.

Qualifications:
  • Bachelor's degree in nursing.
  • A minimum of 2 years of clinical experience in a relevant setting.
  • Active and unrestricted state nursing license, DEA license, and board certification.
  • Current driver's license and proof of auto insurance.
  • Experience in senior patient care is essential.
  • Current Basic Life Support (BLS) certification from a recognized organization.
  • Proficient in using technology for patient care and documentation.
  • Leadership experience in healthcare projects is preferred.

Preferred Qualifications:
  • Master's degree in nursing.
  • Experience with house calls or in-home care.
  • Background in urgent care, primary care, or internal medicine.

Work Schedule:
Full-time position with a standard schedule of Monday to Friday, 40 hours per week.
