Position Overview

Role: Licensed Practical Nurse (LPN) specializing in hospice care.

Responsibilities:

  • Implement the care plan established by the supervising Registered Nurse.
  • Educate patients and caregivers on preventive measures and assist families in recognizing health concerns.
  • Encourage patient and family autonomy in managing health care responsibilities.
  • Deliver compassionate physical, emotional, and spiritual support to patients and their families.
  • Assess the safety of the home environment and suggest necessary improvements to mitigate risks.
  • Apply the nursing process effectively to carry out care plans for patients and their families.
  • Provide immediate care in emergency situations as required.
  • Engage in community health assessments and participate in health education and preventive initiatives.
  • Maintain personal well-being, including physical stamina and mental health.

Qualifications:

  • Graduate from an accredited Practical Nursing program with a focus on pharmacology.
  • 2 to 3 years of recent experience in acute care nursing or a similar field; experience in community health nursing is a plus.
  • Valid LPN license in the State of Maine and current CPR certification.
  • Possession of reliable transportation, insurance, and a valid Maine driver's license.
