Cleveland Clinic is seeking a skilled physician to join our team as a full-time physician in a Skilled Nursing Facility (SNF) position. As a key member of our post-acute care team, you will provide medical oversight and consulting services to patients at two area skilled nursing facilities.
Responsibilities
- Provide assessment and consulting services to patients at two area skilled nursing facilities.
- Collaborate with nurse practitioners and home care teams to ensure seamless care transitions.
- Develop and implement medical direction for the program and facilities, as desired.
- Participate in leadership responsibilities, research activities, education, and clinical program development.
