POSITION OVERVIEW
- Full-time role requiring 40 hours per week, with 24 hours dedicated to patient interactions
- Availability for same-day or next-day patient appointments
- Tasked with managing chronic health conditions and conducting Annual Wellness Visits
- Facilitate smooth transitions of care and engage in a comprehensive onboarding program lasting 21 months

CANDIDATE REQUIREMENTS
- Certification as a family or adult geriatric nurse practitioner (AANP or ANCC) or physician associate (NCCPA) is essential
- Experience in a high-volume primary care environment is preferred
- Ability to manage complex medical cases while delivering exceptional customer service
- Proven capacity to work autonomously as well as collaboratively within a team
