We are seeking a dedicated and compassionate Registered Nurse to join our team at Geisinger Home Health in Hazle Township, Pennsylvania. As a Registered Nurse, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

As a Registered Nurse in Home Health, you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Your primary focus will be on coordinating total plan of care with other health care professionals involved in care and helping to achieve and maintain continuity of patient care.

Responsibilities
  • Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop the plan of care within state-specific guidelines.
  • Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source.
  • Collaborate with physician orders and other healthcare professionals to provide high-quality patient care.
  • Document accurate, complete, and timely information in collaboration with physician orders.
Requirements
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
