Overview:

A leading healthcare organization in the United States is seeking a skilled Geriatric Care Specialist to join their team. This exciting opportunity offers the chance to work with older adults in a home care setting, providing community-based outpatient services under Medicare Part B.

About the Role:

The successful candidate will be responsible for delivering high-quality, personalized care to patients in their homes, focusing primarily on geriatric care. This role requires flexibility and autonomy, allowing the specialist to tailor their work schedule to suit their needs.

Key Responsibilities:

  • Providing 1-on-1 direct patient care to older adults in a home care setting
  • Working collaboratively with a multidisciplinary therapy team to develop and implement effective treatment plans
  • Utilizing evidence-based methods to deliver creative and innovative care solutions
  • Maintaining accurate records and communicating effectively with patients and their families

Requirements:

  • Doctor of Physical Therapy (or equivalent) degree
  • Eligibility for South Carolina licensure
  • For PRN/Per Diem roles, a minimum of 10 hours of direct patient care and 3 weekdays of availability are required
  • Professionalism, accuracy, and dependability
  • Passion for providing high-quality care
