As a Care Coordinator at West Central Behavioral Health, you will partner closely with our Clinical Team to deliver care coordination and case management services for a designated group of clients engaged in community-based and in-office skills-oriented therapy. Your primary responsibility will be to evaluate client needs, organize care, and connect individuals with essential community resources. Additionally, you will support clinical processes related to mental health treatment as necessary.

Key Responsibilities:
  • Assist clients and their families in understanding treatment options and making informed choices through regular check-ins via phone and in-office meetings.
  • Collaborate with the treatment team to ensure seamless coordination of care and information exchange.
  • Contribute to the creation and execution of treatment plans, case management, and other clinical documentation.
  • Encourage effective use of clinical resources, community services, and additional social supports.
  • Guide clients in accessing resources by providing information and assisting with benefit applications when needed.
  • Facilitate care coordination and communication both internally and externally to ensure continuity of services.
Inquire about options for remote/hybrid work, full-time/part-time positions, or job sharing.

Qualifications:

Education:
  • A Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, Human Services, or a related field is required, with a preference for candidates with 2 years of relevant experience.
Experience:
  • Minimum of 2 years of related experience.
Essential Skills:
  • Strong communication, problem-solving, time management, and organizational skills are essential.
  • Flexibility and the ability to work independently with minimal supervision are required.
  • Proficient computer skills and familiarity with Microsoft Office are necessary.
  • Capability to learn essential computer and technical skills pertinent to the role and organizational needs.
  • Ability to engage with staff, clients, and the community within a mental health framework.
  • Maintain competencies in customer service, quality improvement, compliance, and safety.
Preferred Skills:
  • Familiarity with electronic medical record systems is preferred.
  • Experience in providing targeted case management services.
  • Prior experience in a community mental health setting is advantageous.
  • Knowledge of evidence-based mental health treatments such as Dialectical Behavioral Therapy (DBT), Cognitive Behavioral Therapy (CBT), Enhanced Illness Management and Recovery, and Prolonged Exposure for PTSD is beneficial.

  • Ability to access all Agency locations.
Additional Requirements:
  • Willingness to travel as necessary for job responsibilities.
  • Possession of a valid state driver's license.
  • Access to reliable transportation.
  • Maintain personal automobile insurance.
  • Ability to obtain and sustain Agency and/or State of New Hampshire required background checks.
  • Understanding of confidentiality importance and the ability to uphold it.
