Job Summary

We are seeking a skilled Hospice RN for our Home Health team. As a Hospice Nurse, you will provide high-quality patient care and support in the comfort of their own homes.

Key Responsibilities
  • Lead and collaborate with the care team to develop and implement patient care plans
  • Support patients in the Carnegie/Crafton, PA area and surrounding regions
  • Drive to patient locations, with approximately 20% of the day spent on travel
  • Meet critical deadlines for charting and maintain accurate records
  • Self-manage time and schedule to ensure efficient patient care
Schedule and Shift Details

As a Hospice RN, you will be required to work on-call Friday, Saturday, and Sunday from 5:00pm to 10:00pm, with additional units on Saturday and Sunday. You will also be required to work 2 holidays per year.

Requirements
  • Associate degree or higher in Nursing
  • 1+ years of experience in Clinical Documentation, Clinical Informatics, Clinical Quality, or Electronic Medical Record (EMR)
  • Valid Registered Nurse (RN) license in Pennsylvania (PA)
  • ACLS or BLS certification via American Heart Association
  • Ability to travel locally and drive to patient locations
