Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at Hospice of the Western Reserve. As a LPN, you will provide high-quality patient care in a variety of settings, collaborating with a transdisciplinary team to deliver comprehensive care.

About the Role

This is a 36-hour per week position, working 8pm-8am every third weekend and holiday. The ideal candidate will have experience in hospice care, home health, oncology, and/or pain and symptom management.

As a LPN on our team, you will:

  • Practice in accordance with HWR clinical standards of practice, policies, and procedures
  • Assess physical, psychosocial, and spiritual needs of patients and families/caregivers
  • Provide intermittent skilled nursing services under physician direction
  • Monitor and evaluate pain and symptoms, providing information and assistance to patients and families
What You Will Do

In this role, you will be responsible for providing emotional support to patients and families, referring them to other services when necessary, and maintaining accurate records and reports.

You will also participate in care conferences, bereavement follow-up, case review, quality improvement activities, and other meetings as assigned.

Requirements

To be successful in this role, you must have:

  • Licensed in the State of Ohio
  • Experience in hospice care, home health, oncology, and/or pain and symptom management
  • Current CPR certification
  • A valid driver's license, good driving record, and auto accident insurance

Hospice and Palliative Care Certification is desired but not required. We offer equivalent education/experience may substitute for minimum qualifications.
