As an Advanced Practice Clinician on our team, you will play a critical role in delivering comprehensive and interdisciplinary primary care to patients. About the Role

The Hopscotch Care Team is responsible for delivering high-touch, high-quality care to patients in rural areas. As an Advanced Practice Clinician, you will be part of this team, working closely with physicians, community health workers, case managers, and other healthcare professionals to ensure seamless coordination of care.

Your key responsibilities will include:

  • Providing comprehensive primary care services to a set panel of patients, including office visits and telehealth visits for routine and acute primary care issues
  • Working closely with the care team to develop and implement personalized care plans that address the unique needs and goals of each patient
  • Engaging patients and caregivers in education and coaching to promote healthy behaviors and self-management of chronic conditions
  • Collaborating with other healthcare professionals to leverage best practices and evidence-based approaches in care delivery
Requirements

To be considered for this role, you must have:

  • A minimum of two years' experience in a primary care specialty (internal medicine, geriatrics, family medicine)
  • Experience working with multiple software platforms, electronic health records, and data processing systems
  • An active, non-probationary state medical license
  • US work authorization

PREFERRED QUALIFICATIONS:

  • Experience with the Medicare population, including in-home care as well as documentation and compliance requirements
