Job Summary:

We are seeking a compassionate and skilled Physical Therapist to join our team at Relient Health. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to our residents in a great Assisted Living Facility (ALF) / Memory Care setting.

Responsibilities:
  • Screen patients and perform evaluations to develop initial and interim treatment plans
  • Document progress toward identified objectives and oversee Physical Therapy Assistant(s)
  • Develop and implement new programs/techniques to enhance physical therapy services
  • Conduct patient and family education and deliver quality physical therapy services
  • Put patient service first and work collaboratively as a team player
Qualifications:
  • Graduate of an accredited Physical Therapy program
  • State licensure or eligibility
  • Stable Work History and positive attitude
  • Skilled Nursing Facility (SNF) experience preferred
