About the JobIn this role, you will work collaboratively with our interdisciplinary team to assess patients' needs, develop and implement individualized care plans, and provide direct patient care. Your expertise and dedication will help us achieve outstanding clinical outcomes and make a meaningful difference in the lives of our patients.
Responsibilities and Requirements- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
- Collaborate with healthcare professionals to ensure seamless transitions of care and optimal patient outcomes.
- Must have a valid LPN/LVN license in good standing and at least six months of hands-on nursing experience, preferably in home care or a related setting.
