Home Health Registered Nurse Opportunity

Are you a dedicated and compassionate Registered Nurse looking for a new challenge in home health care? Interim HealthCare, a leading employer of Registered Nurses, is seeking a skilled Home Health Registered Nurse to join our team.

Key Responsibilities:

  • Provide high-quality, patient-centered care to individuals in their homes, promoting independence and well-being.
  • Work collaboratively with a multidisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments, document progress, and report changes to physicians.
  • Educate patients and their families on disease management, medication, and home safety.
  • Ensure seamless transitions between care settings, promoting continuity of care.

Requirements:

  • Active Registered Nurse (RN) License in Colorado.
  • Graduate of an accredited school of nursing.
  • At least 1200 hours of experience as an RN.
  • Healthcare CPR/BLS Certification.
  • Home health care experience preferred.
  • Pass a background check and drug test.
