Pediatric Home Health Care LPN Opportunity

Aveanna Healthcare, a leading provider of pediatric home health care, is seeking a compassionate Licensed Practical Nurse (LPN) to join our team. As a LPN, you will provide skilled nursing care to patients in the comfort of their own homes, making a meaningful difference in the lives of children and families.

Job Summary:

We are seeking a dedicated and experienced LPN to provide high-quality patient care in a home health care setting. The ideal candidate will have a strong background in pediatric nursing and excellent communication skills. As a member of our team, you will have the opportunity to work with a diverse range of patients and families, and contribute to the delivery of exceptional patient care.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Collaborate with healthcare teams to ensure seamless care coordination and communication
  • Document patient care and progress in a timely and accurate manner

Requirements:

  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures

Preferred Qualifications:

  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
