Job Summary:

We are looking for a highly motivated and experienced Registered Nurse to coordinate the delivery of health services that will impact beneficiaries' health, well-being, and quality of life. The successful candidate will implement and manage innovative medical programs that support the medical management principles of the Funds.

Key Responsibilities:
  • Conduct nursing assessments in the homes of beneficiaries to coordinate health services and improve health outcomes.
  • Collaborate with providers and community agencies to promote beneficiary welfare and health education.
  • Develop and implement health promotion, disease prevention, and health education programs.
  • Network with vendors to develop health fairs and events that promote beneficiary access and participation.
  • Conduct health education seminars for beneficiaries.
  • Identify health care issues affecting beneficiaries and implement case conferences with the medical director and/or medical management vendor.
  • Develop and maintain effective working relationships with field health and medical management staff, and other medical professionals, beneficiaries, their families, and community agencies.
  • Prepare written reports and/or oral presentations to trustees and/or operations staff on the status of activities in the clinical field service office.
  • Record all beneficiary-related interactions timely in the Fund's designated medical management system.
Requirements:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations might be made to enable individuals with disabilities to perform the essential functions.

  • Bachelor's degree in nursing from an accredited college/university plus five years' experience in nursing. Three years in community health nursing desirable.
  • Associate degree in nursing or graduation from a hospital diploma program plus 5 years home health experience and/or geriatric outpatient clinic. A certificate in case management recommended.
  • Current Registered Nurse (RN) licensure.
Technical Business Industry Knowledge:

Current knowledge of nursing methods and practice, including community health nursing principles. Knowledge of managed care principles, health care management, case management, and disease management. Also, must be cognizant of the health care needs of women and geriatric patients.
