**Job Summary**
We are seeking a real leader who has the passion, commitment, talent, and ability to work collaboratively with their peers to advance our mission and vision. As a Community Mental Health Practitioner, you will have the pleasure of providing psychosocial support to individuals, families, or vulnerable populations coping with chronic, acute, or terminal illnesses and physical or developmental disabilities.
**Key Responsibilities:**
- Provides patient education and counseling
- Advises caregivers and makes necessary referrals for other social services
- Completes bio-psychosocial assessments and collaborates with consumers to complete individualized treatment plans
- Conducts individual, group, and family therapy sessions
**What We Offer:**
- Competitive salary: $45,000 - $65,000 per year, based on experience
