About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Hospice team as a Case Manager. In this role, you will provide high-quality patient care and support to individuals and their families, promoting dignity and comfort throughout the dying process.

Key Responsibilities
  • Provide skilled nursing care and case management to clients and their families, in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice.
  • Develop and implement individualized care plans, in collaboration with the interdisciplinary team, to meet the unique needs of each client.
  • Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination and support.
  • Assess and manage pain and symptoms, utilizing evidence-based practices and interventions to promote comfort and quality of life.
  • Document all interactions, assessments, and interventions in a timely and accurate manner, in accordance with regulatory requirements and organizational policies.
Requirements
  • Graduate of an approved, accredited school of professional nursing required; BSN preferred.
  • Current licensure to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
  • Current CPR certification required.
  • Minimum of one (1) year medical/surgical nursing experience required; hospice/home care experience preferred.
  • Demonstrated excellent verbal and written communication skills required.
  • Demonstrated strong interpersonal, organizational, and time management skills required.
