About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Community Rehabilitation Hospital North. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our rehabilitation programs.

Key Responsibilities
  • Assess and reassess patients to identify their individual needs and develop effective care plans.
  • Administer medications and treatments as prescribed by physicians and registered nurses.
  • Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes.
  • Collaborate with interdisciplinary teams to provide comprehensive care and support to patients and their families.
  • Participate in quality improvement initiatives to enhance patient care and safety.
Requirements
  • Graduate of an accredited Licensed Practical Nurse program.
  • Current state licensure as a Licensed Practical Nurse.
  • CPR/BCLS certification and ACLS preferred.
  • IV Certified or obtained within 6 months highly preferred.
  • Minimum six months' experience in a medical/surgical setting, preferably in acute care.
