We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Autumn View Health Care Facility in Hamburg, NY. As an LPN, you will play a vital role in promoting the health, wellness, and illness prevention of our residents.
Key Responsibilities- Ensure the care and safety of all residents in the nursing unit
- Administer and document medications, treatments, and other nursing services
- Maintain accurate records of resident conditions and provide timely updates to the nursing team and medical providers
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless care delivery
- Participate in the orientation of new staff and maintain competency in all tasks required by CMS
- Minimum 18 years of age
- New York State (LPN) Licensed Practical Nurse licensure
- Strong written and verbal communication skills
- Basic computer knowledge and PCC e-MAR experience preferred
- Physical endurance for frequent activity and ability to lift a minimum of 20 pounds
