PruittHealth is seeking a highly skilled Occupational Therapist to join our Home Health team. As an Occupational Therapist, you will play a vital role in helping patients achieve their maximum potential and independence in the comfort of their own homes.
Key Responsibilities- Conduct comprehensive evaluations and assessments to identify patient needs and develop individualized care plans.
- Provide occupational therapy services to patients in their homes, focusing on functional skills and activities of daily living.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Stay up-to-date with industry developments and best practices through ongoing education and training.
- Valid professional OT license in the state of practice.
- Graduate of an ACOTE-accredited school and eligible to take or has passed the NBCOT exams.
- Ability to work independently and as part of a team.
