Vascular Surgery Nursing Care Coordinator

We are seeking an experienced Clinical Nurse to coordinate and deliver exceptional patient care in our vascular surgery department. As a key member of our team, you will play a vital role in assessing patient needs, developing and implementing care plans, and collaborating with other healthcare professionals to ensure seamless care coordination.
Your primary responsibilities will include:
- Delivering and coordinating patient care with the caregiving team to achieve optimal patient outcomes.
- Implementing physician orders, administering medications, and performing treatments as required to meet patient needs.
- Assessing and evaluating patient responses to care rendered and making adjustments as necessary.
- Collaborating with the nursing team to create a Plan of Care for all patients and ensuring its effective implementation.
- Demonstrating expertise in clinical knowledge, technical skills, and interpersonal relationships to provide high-quality patient care.
