Interdisciplinary Healthcare Professional

We are seeking a highly skilled Clinical Social Worker I to join our team at McLean Hospital(MCL). This role is ideal for individuals who are passionate about delivering exceptional patient care and have excellent communication and collaboration skills.
\As a member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide comprehensive psychosocial assessments and develop effective treatment plans. You will be responsible for evaluating patients' medical or physical condition, assessing client needs, and working collaboratively with the healthcare team to develop and implement treatment plans that support patient-centered care.
\Key Responsibilities:

- Evaluate patients' medical or physical condition and assess client needs as part of the interdisciplinary team\
- Develop and implement treatment plans that support patient-centered care\
- Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan\
- Provide psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues\
- Formulate biopsychosocial assessment, disposition, and treatment plans\
- Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team\
- Maintain accurate and up-to-date records of client progress and treatment plans\
- Promote collaboration and equity among healthcare team members and organization to support and enhance patient care\
- Participate in departmental initiatives and promote opportunities for growth and development within the organization\
- Stay up-to-date with industry developments and best practices in mental health and behavioral medicine\\
Requirements:

- Master's Degree in Social Work (MSW) or related field required\
- Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) preferred\
- Excellent communication, collaboration, and problem-solving skills\
- Ability to maintain confidentiality and handle sensitive information\
- Flexibility to adapt to changing priorities and deadlines\
- Knowledge of mental health and behavioral medicine principles and practices\\
