We are seeking a compassionate and dedicated Registered Nurse (RN) to join our team as a PRN Home Health and Hospice nurse. This is a flexible, as-needed position, perfect for individuals looking for a convenient work schedule while making a meaningful impact on patients' lives.
Job Responsibilities:- Assess and coordinate patient discharge planning needs with healthcare team members.
- Perform point-of-care testing according to policies and procedures.
- Collaborate with physicians regarding the plan of care and patient status, providing regular updates and recommending revisions as needed.
- Coordinate and communicate with multi-disciplinary team members and outside agencies to achieve expected outcomes for patient care and education.
- Complete admission paperwork and verify insurance information, ensuring timely and accurate documentation.
- Provide skilled nursing care within the established plan of care, including tasks such as taking vitals, performing wound care, IV care, ostomy care, blood draws, injections, and physical assessments.
- Document visit notes, treatments, and care, ensuring timely and accurate completion in adherence to regulations.
- Collaborate with patients, families, and healthcare teams to develop comprehensive plans of care based on unique patient needs.
- Respond to patient, family, and healthcare team inquiries, completing follow-up to address needs, issues, or concerns.
- Graduate of an accredited school of nursing or education equivalency for licensing.
- No experience required.
- Constant use of speech to share information through oral communication.
- Constant standing and walking.
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of smell to detect/recognize odors.
- Frequent use of hearing to receive oral communication, distinguish body sounds, and/or hear alarms, malfunctioning machinery, etc.
- Frequent driving.
- Frequent keyboard use/data entry.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Occasional lifting/moving of patients.
- Occasional bending, stooping, kneeling, squatting, twisting, gripping, and repetitive foot/leg and hand/arm movements.
- Rare crawling and running.
- Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA).
- Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR).
- Driving Record - Motor Vehicle Report (MVR).
- Driver License - State Issued.
