We are seeking an experienced and organized Clinical Nurse Consultant to manage the clinical care needs of residents. This is a challenging yet rewarding role that requires autonomy, efficiency, and strong communication skills.

Responsibilities:
  • Complete Minimum Data Set (MDS) assessments within regulated timeframes.
  • Evaluate resident care needs through physical examination, interviews, and chart reviews.
  • Discuss resident care requirements with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
  • Review information from hospitals, consults, and outside agencies to inform assessment and care planning.
    • Coordinate significant changes to MDS assessments.
    • Facilitate Prospective Payment System (PPS) meetings and maintain PPS boards for monitoring Medicare days and Resource Utilization Groups (RUGs) utilization.
  • Stay current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
Requirements:
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) certification.
  • AANC certification (RAC-CT) is highly valued.
  • Knowledge of RAI process, including PPS principles.
  • Experience as an MDS Nurse is preferred.
