We are seeking a compassionate and enthusiastic Registered Nurse (RN) with assessment experience in long-term care to join our team at Laurel Lakes Rehabilitation and Wellness Center.

About the Role

As a Registered Nurse Assessment Coordinator, you will be responsible for conducting and coordinating the completion of resident assessments in accordance with state requirements and facility policies. You will also develop and implement quality assurance programs, participate in facility surveys, and provide education to the interdisciplinary team.

Requirements

To be successful in this role, you will need to be a Registered Nurse with effective leadership and assessment skills in a long-term care setting. You must also be proficient in PA CMI and able to pass a post-offer, pre-employment criminal background check and physical/drug screen.
