Job Summary:

The successful candidate will provide community-based outpatient services (Medicare Part B) to homes and/or within Senior Living Communities in the area.

Responsibilities:
  • Provide physical therapy services to patients in their homes and/or within Senior Living Communities.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive care.
  • Maintain accurate records and documentation of patient progress.
  • Participate in ongoing education and training to stay current with best practices.
Requirements:
  • Graduation as a Doctor of Physical Therapy (or equivalent standing).
  • Eligibility for Alabama state licensure as a Physical Therapist.
  • Availability to work 10 hours per week.
  • Regular and consistent weekly availability is required, which includes 3 weekdays of availability.
