About the Role

The successful candidate will be responsible for conducting comprehensive assessments and evaluations of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. You will also be responsible for developing and implementing individualized therapy plans, using evidence-based techniques, in coordination with other members of the healthcare team.

  • Conduct thorough assessments and evaluations of patient care needs.
  • Develop and implement individualized therapy plans.
  • Provide ongoing support and guidance to patients and their families.
