Responsibilities:
The Registered Nurse Case Manager will be responsible for providing care and services to a group of patients, visiting them in their homes, skilled nursing facilities, and assisted living facilities. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Requirements:
To be successful in this role, you will need to have a current Registered Nurse (RN) licensure in the state of Georgia, with previous hospice experience. You will also need to be able to work effectively in a team environment, manage your time efficiently, and prioritize tasks to meet deadlines. Additionally, you will need to be able to learn and utilize electronic medical records systems and have a valid driver's license and reliable transportation.
