Job Summary:

This position provides high-quality care to assigned clients in a compassionate and efficient manner, adhering to location standards, procedures, and individualized client plans of care.

Key Responsibilities:
  • Define the unit of care (family and client) and demonstrate knowledge of hospice palliative care, philosophy, and scope of services.
  • Recognize different cultural attitudes and values towards death and dying, and identify the five stages of the grieving process, anticipatory grief, normal vs. abnormal grief, and impact on client's family.
  • Provide care according to plan of care, evaluate clients' need for additional services, and involve client and family in creation of plan of care.
  • Coordinate with interdisciplinary team as needed, complete required documentation in appropriate timeframes, and explain advance directives, clinical requirements, and interdisciplinary services available through agency.
  • Work independently with minimal supervision, seek guidance as needed, and perform initial client assessment and ongoing assessments of client's status.
  • Participate in creation of plan of care, ensure plan of care is implemented by interdisciplinary group, process physician services/orders, and complete quality assurance audits as assigned.
  • Participate in agency in-services, educational opportunities, team meetings, and committees, and assist with customer experience/marketing activities to expand community reach.
Basic Responsibilities:

This position will be held accountable for complying with all related laws, regulations, company policies, and procedures pertaining to their position and for fulfilling their obligations under the Facilities Corporate Compliance Program.

Qualifications:
  • Graduate of an accredited school of nursing.
  • Licensure for state of practice is required, CPR through AHA.
  • Must be at least 18 years of age.
